Transition from School forms part of our Employment Services.
The transition programme is based on the understanding that the
first option for school leavers is the securing of paid
employment, sometimes via work trials or further tertiary
education.
Students need to be able to make an informed choice about what
career path they would like to follow, often in different
working environments.

How to access our service
A Gracelands transition worker will visit your school and talk
with the Principal and staff as required.
A referral to Gracelands is required and can come from a number
of sources such as yourself, family, Principal, teachers, Health
Professional etc.
Once the referral process has been completed we can begin
working with you. This referral process can take from 2 to 3
weeks.
The service is free!
There is no direct cost to you or your family, if you wish to
access our Transition programme. Your transition from school is
funded by the Ministry of Social Development, so we do need
confirmation from them regarding your eligibility to use our
service. However, we will do all that. If you have any further
