Client Pathway Team

The Client Pathway Team is integral to Gracelands Group of Services. The team is the first point of contact for new enquires and referrals into our services.

WHAT OUR SERVICE PROVIDES:

- A single point of referral into our service
- Individualised vocational planning
- Access to Gracelands programmes and activities
- A flexible services offering continuing support for clients
- A client centred /value focused service
- Information from families and carers.

HOW TO ACCESS OUR SERVICE:

The Client Pathway Team is based at the National Service Centre in Te Awamutu. We are available to undertake referral meetings at any Graceland facilities in Hamilton, Te Awamutu, Cambridge, and Taumarunui. Please contact us if you have any queries about any of our services.
